Your membership entitles you to vote at PTA General Membership meetings and one Free Directory will be given to each family whose adult member(s) join CVMS PTA by October 9, 2009.

Each CVMS student will be listed in the Directory, UNLESS permission is denied on page 83 of the 2009-2010 Student Handbook/Code of Conduct upon its return to Canyon Vista. If there is any information entered below that you do NOT want to appear in the Directory, please indicate which information in the field [NOT LIST] by typing the corresponding code(s) as listed.
